Prerequisites
- You must be logged into your workspace.
- Any role can submit safety reports (pilot, crew, safety_manager, admin, or viewer).
Report types
Before you begin, identify which report type best fits your situation:| Type | When to use |

| Hazard | A condition that could foreseeably cause or contribute to an accident |
| Incident | An occurrence that affected or could have affected the safety of operations |
| Near-miss | An event that did not result in injury or damage but had the potential to do so |
| Concern | A general safety concern that does not fit other categories |
| Observation | A safety-relevant observation from normal operations |
Steps
Open the sidebar, confirm you are in the Safety module, and click Reports. Then click the New Report button in the top-right corner of the reports list.
You can also use the keyboard shortcut
N R to start a new report, or open the command palette with Cmd+K and select New Safety Report.Choose the appropriate Report Type from the dropdown (hazard, incident, near-miss, concern, or observation). Then select a Category that best describes the area of operations involved, such as flight operations, maintenance, ground handling, or cabin safety.
Provide a clear, concise title between 10 and 200 characters. The title should summarize the event or condition in plain language.
Describe what happened (or what was observed) in detail. The description must be between 50 and 10,000 characters. Include:
You can use the voice input button to dictate your description instead of typing. The rich text editor also supports formatting such as bold, lists, and headings.
If you are comfortable making an initial assessment, select the Severity (1-5, from Negligible to Catastrophic) and Likelihood (A-E, from Improbable to Frequent). The risk matrix will display the resulting risk level.
AI-suggested risk levels may appear based on your description. These are advisory only — you must confirm or adjust them before submission.
Anonymous reports cannot be followed up with the reporter. If additional details may be needed during investigation, consider using the Confidential option instead.
Auto-save
Drafts are automatically saved every 60 seconds. If you navigate away before submitting, your draft will be preserved and accessible from the Reports list.Offline submissions
If you lose network connectivity while filling out a report, PlaneConnection will queue your submission locally and sync it when connectivity is restored. An offline indicator will appear in the form to confirm this behavior.Tracking your report
After submission, you can track the status of your report by navigating to Reports in the sidebar or using the tracking code provided at submission. Go to Reports > Track and enter your tracking code to view the current status without needing to log in.Related
