Safety reports are the foundation of your Safety Management System. PlaneConnection supports multiple report types and confidentiality levels to encourage open reporting across your organization.

Prerequisites

  • You must be logged into your workspace.
  • Any role can submit safety reports (pilot, crew, safety_manager, admin, or viewer).

Report types

Before you begin, identify which report type best fits your situation:

Near-missAn event that did not result in injury or damage but had the potential to do so
ConcernA general safety concern that does not fit other categories

Steps

Open the sidebar, confirm you are in the Safety module, and click Reports. Then click the New Report button in the top-right corner of the reports list.

You can also use the keyboard shortcut N R to start a new report, or open the command palette with Cmd+K and select New Safety Report.

Select the report type and category

Choose the appropriate Report Type from the dropdown (hazard, incident, near-miss, concern, or observation). Then select a Category that best describes the area of operations involved, such as flight operations, maintenance, ground handling, or cabin safety.

Enter the report title

Provide a clear, concise title between 10 and 200 characters. The title should summarize the event or condition in plain language.

Write the description

Describe what happened (or what was observed) in detail. The description must be between 50 and 10,000 characters. Include:

  • What occurred and when
  • Where the event took place (airport, airspace, facility)
  • Who was involved (roles, not names, if submitting anonymously)
  • Environmental conditions if relevant (weather, time of day, lighting)

    You can use the voice input button to dictate your description instead of typing. The rich text editor also supports formatting such as bold, lists, and headings.

    Assess initial risk (optional)

    If you are comfortable making an initial assessment, select the Severity (1-5, from Negligible to Catastrophic) and Likelihood (A-E, from Improbable to Frequent). The risk matrix will display the resulting risk level.

    AI-suggested risk levels may appear based on your description. These are advisory only — you must confirm or adjust them before submission.

    Attach evidence (optional)

    Click the file upload area to attach supporting evidence such as:

  • Photos or screenshots
  • Scanned documents
  • Audio recordings
  • Supporting data files

    Files are stored in encrypted R2 storage associated with your workspace.

    Choose your confidentiality level

    You have three options:

  • Identified: Your name and contact information are visible to safety managers.
  • Confidential: Your identity is encrypted (AES-256-GCM) and only accessible to the accountable executive. Safety managers see the report content but not your identity.
  • Anonymous: No identifying information is stored with the report. This option cannot be reversed.
  • 22
    Anonymous reports cannot be followed up with the reporter. If additional details may be needed during investigation, consider using the Confidential option instead.

    Review and submit

    Review all entered information on the summary panel. When satisfied, click Submit Report.

    After submission:

  • A unique report number in the format RPT-YYYY-NNNNN is assigned.
  • A QR code is generated for tracking the report.
  • If you provided an email address and chose an identified or confidential report, you will receive a confirmation email.
  • Auto-save

    Drafts are automatically saved every 60 seconds. If you navigate away before submitting, your draft will be preserved and accessible from the Reports list.

    Offline submissions

    If you lose network connectivity while filling out a report, PlaneConnection will queue your submission locally and sync it when connectivity is restored. An offline indicator will appear in the form to confirm this behavior.

    Tracking your report

    After submission, you can track the status of your report by navigating to Reports in the sidebar or using the tracking code provided at submission. Go to Reports > Track and enter your tracking code to view the current status without needing to log in.
