By the end of this guide, you will have completed a FRAT evaluation for a flight — scoring risk factors, reviewing the composite risk level, documenting mitigations, and submitting the assessment for your go/no-go decision.
The Operations module must be enabled for your workspace. You also need the appropriate operations permissions. Contact your workspace administrator if you cannot access these features.
You need access to the Operations module. A trip must exist to link the FRAT to it, though standalone FRATs can be submitted without a linked trip.

Complete the FRAT

Step 1: Open the assessment form

Navigate to the FRAT from the Operations sidebar, the Trip detail page, the My Cockpit quick-action, or the Command Center. If you open it from a trip, the flight details are pre-populated.

Step 2: Evaluate each risk factor

Work through each expandable section — Weather, Crew, Aircraft, and Operational. For each factor, select the rating that best describes the current conditions. Each section displays a subtotal as you go.

Be honest in your assessment. Understating risk to produce a lower score masks conditions that may warrant additional mitigations or a no-go decision.

Step 3: Review the composite score

After completing all factors, a composite score (0—25) appears with a color-coded risk level. If you receive a High-level score (above 17), obtain explicit authorization from the chief pilot or accountable executive before the flight can proceed.

Step 4: Document mitigations (if needed)

If your score is Medium or High, describe the specific actions you will take to reduce risk in the mitigations text field — for example, delaying departure, adding a second pilot, selecting an alternate route, or deferring to a more experienced crew.

Step 5: Submit the assessment

Click Submit. The assessment is linked to the associated trip (if applicable) and the submitting pilot’s profile.
